Medical Facility Notary / Advance Directives Advance Directives have two parts Medical Power of Attorney : Choosing and appointing a Healthcare Representative and Living Will : Deciding your medical treatment options if you become incapacitated. 1-) You, as the Principal, will need to elect a "Proxy Agent" or "Health Care Representative", choosing and appointing the person who will oversee your medical decisions for you. Particularly useful in case of an accident or sudden incapacitation or simply at a very old age. 2-) Living Wills pertain to treatment options: Do you wish to be placed on a ventilator? artificially nourished? What are you personal religious beliefs in case you pass away etc... Most people oversee making early Advance Directives, but anything can happen at anytime and for any reason. If your Advance Directives are clearly defined prior to a severe medical issue, this alleviates the burden on the part of your family to make choices for you, you would not otherwise have agreed with and avoid family conflicts.
Certified Copies : School Records, Transcripts, Medical records, there are many instances when a certified copy of a document is requested. A copy of the document shall be provided by the Custodian of the document (the keeper of the original document), the Notary will compare both copies and attach to the copy a notarial certificate stating the copy is legitimate. One other option for document certification is to have you sign an affidavit in which you state that the document is an original or a true copy. Affidavit that we will notarize.
Power of Attorney A power of attorney (POA) is a written instrument granting another person the authority to act on your behalf. The person to whom you grant this authority is referred to as your attorney-in-fact. As the person granting the authority, you are the principal. A POA may be used to purchase property, managing financial accounts, Health Care Decisions, property management, filing taxes, investment decisions etc... POAs come in many forms. There two main types of POAs (but not restricted to): Durable Power of Attorney and General Power of Attorney. The General Power of Attorney is valid until you terminate or revoke it - or - until you become incapacitated, or you die. A Durable Power of Attorney on the other hand, extends the attorney-in-fact, the right to act on your behalf even if you are incapacitated. Any type of POA is a powerful tool and the authority it confers your attorney-in-fact must be carefully considered.
Affidavits There are many situations where an affidavit can be needed. For example, if your identity has been stolen and you need to prove that you are you. Or if you need for evidence to be included in a Penal or Civil Court Hearing or if you have to outline your financial status, or for a name change etc... See it as a written sworn statement. And notarized.
Living Trusts - Wills and Testaments Declaration of Trusts are generally drafted by your Attorney. LCSA Notary Services will deliver and present Trust documents to all concerned parties, while abiding by your directives with Beneficiaries. We prefer to work closely with attorneys and other trust professionals and will travel to their office before Presentation. Once the documents have been presented and delivered, we will ensure that any documents you require to be returned to your office are promptly returned by private courier or FedEx/UPS or simply driven onto location if requested. Premarital & Prenuptial Agreements This is essentially about protecting your assets and defining what would happen in case of a separation. This process is particularly useful to avoid expensive legal fees in case of a Divorce. And a pre-marital agreement also serves to establish assets accumulated - during - the marriage. A lot of information can be found on the internet regarding the topic and a Public Notary is not an Attorney at Law. But it is highly advisable to seek the services of a Family/Estate Attorney who will assist all parties in drafting the agreement. Such document however needs to be notarized in the state of New Jersey.
Guardianship & Adoption Documents A guardianship and custody document must be notarized to be legally binding. The topic is complex and it is advisable to get guidance from a specialized attorney.
Employee Verification Form (I-9) Let us help you verify the employment eligibility status of your newly hired remote workers. We will examine the identity and employment authorization documents for their prima facie value. For Executive recruits, we can travel to them. We can meet them at their homes, hotels, remote offices, or even at the airport. We will archive all completed I-9 forms, as well as copies of identity and employment authorization documents.

NOTARY LOAN SIGNING AGENT (NSA)

In addition to our loan signing notary services, we can also directly pick up and drop off your documents at your local escrow office or other location.
Loan signings are scheduled at either your home or agent office.
Whether you are a real estate agent, lender, title company, escrow, or an individual in need of notary services, we can handle all your loan signing needs with care and expertise.
The typical steps on the part of the loan signing agent involve :

Coordinate appointment times with the borrower.

Help borrowers locate (only) specific pages within a loan signing package.

Notarize the Loan Signing.

Print the "critical documents" which only require a notarized signature for the title or escrow company.

Return all the documents (by express courier or in person) to the loan signing to the title company. If required.
Note : a notary pubic acting as a signing agent is not a closing agent, a banker, or a lawyer. We cannot answer questions about or change the terms of the loan or answer any questions about a specific loan. These questions must be addressed by your loan signing representative.

Like any other type of POA, the Medical Power of Attorney grants someone else the authority to make healthcare decisions on your behalf. Particularly important for End of Life Care choices.
LCSA Notary Services can travel to your medical facility and notarize such document. As an ex End of Life Care Chaplain myself, I always advise my Loved Ones to Notarize Advance Directives - before - a medical situation arises. This is something you may wish to discuss with your Doctor. The more specific your Advance Directives are, the better your medical wishes will be respected.
In NJ a DNR must also be signed by your Doctor.
